What is MilliExo® System for Autologous Exosomal Activation?

MilliExo® is an advanced system that uses the patient’s own adipose tissue to obtain a micro-filtered preparation rich in mesenchymal stromal/stem cells (MSCs) and high-potential regenerative sub-populations. The sample is processed and then incubated under controlled conditions of hypoxia and physiological temperature (~37 °C), stimulating the secretion of functional exosomes.

The result is an autologous biological suspension, standardised and ready for use in aesthetic and regenerative medical protocols.

What is MilliExo Lab® Device for Exosomal Activation?

MilliExo Lab® is the medical device dedicated to the exosomal activation phase of the adipose microfiltrate obtained with the MilliExo® kit. The device maintains:

  • controlled hypoxic conditions;
  • constant physiological temperature at approximately 37 °C;
  • standardised incubation times.

This maximises the production of exosomes whilst preserving cell viability and the stability of the preparation.

Difference Between MilliExo® Kit & MilliExo Lab® Device

MilliExo® refers to the complete procedural protocol and the final outcome: an autologous adipose microfiltrate rich in MSCs, subjected to a physiological exosomal activation process.

MilliExo Lab® is the device that provides the ideal conditions (hypoxia, controlled temperature, and standardised timing) to stimulate exosome secretion and make the suspension clinically ready.

Origin of Exosomes in the Autologous MilliExo® Preparation

The exosomes obtained with MilliExo® originate exclusively from the patient’s own adipose tissue. The fat is harvested using a minimally invasive technique and processed through a patented micro-filtration system, allowing:

  • preservation of viable MSCs;
  • maintenance of CD34⁺ and CD146⁺ sub-populations;
  • retention of pericytes and active stromal cells.

During the subsequent physiological incubation phase, these cells naturally and intensively release regenerative exosomes.

Clinical Benefits of Controlled Exosomal Activation

Activation performed under hypoxia and physiological temperature promotes enhanced release of:

  • pro-angiogenic factors;
  • regenerative microRNA;
  • proteins and paracrine mediators.

Is a Centrifuge or External Laboratory Required for MilliExo®?

No. The MilliExo® protocol has been designed to be performed entirely in a closed circuit, without the need of:

  • centrifuges;
  • cell cultures, or drugs;
  • processing outside the sterile field.

The entire procedure can be carried out in real time, in an outpatient setting or operating room, with a final product that is exclusively autologous.

Duration of the MilliExo® Autologous Exosome Procedure

The complete procedure generally takes around 45 minutes, divided into:

  • adipose tissue harvesting;
  • selective micro-filtration using the MilliExo® kit;
  • exosomal incubation in the MilliExo Lab® device.

At the end of the process, the exosomal suspension is ready for use during the same session, without additional downtime.

Cells Preserved in the Adipose Microfiltrate

The selective filtration system preserves:

  • MSCs with CD44⁺/CD90⁺/CD73⁺ phenotype;
  • CD34⁺ sub-populations with high regenerative potential;
  • CD146⁺ pericytes and viable stromal micro-fragments.

While reducing or removing:

  • fibrotic extracellular matrix;
  • lipid residues;
  • large adipocyte clusters;
  • undesired pro-inflammatory components.
